I understand that ELITE is not a salon or service providing entity, Hair Stylists & Makeup Artists are (Self Employed) Independent Contractors and are not Employed or Employees of ELITE. Hair Stylists and Makeup Artists reserve the right to refuse service to anyone if they feel their lively hood or safety is in any way shape or form in jeopardy and/or in an Unsafe/Insufficient work environment to perform services. Stylists / Makeup Artists advertised on ELITE.net are in agreement to abide by governed law, state and local regulations and policies to perform their independent services however ELITE is not responsible for obtaining such credentials and is not held liable for insufficient regulatory requirements. ELITEBEAUTE.NET is a commission based online referrer of services to individuals under an independent agreement.
Release of Liability:
I agree and hereby release from all liability and hold harmless ELITE and any of its referred Independent contractors / Hair Stylists and or Makeup Artists representing or related to ELITE. This liability release is for any and all liability for personal injuries including death and property losses or damage in connection with any activity or accommodation of the above mentioned Business.
Credit Card Payment Authorization:
I agree to this form and I give permission to debit or charge my Valid Credit Card or Debit account for the amount 30% of service totals on or after the date of online submission to secure my reservation. This is permission for that I certify, I am the authorized user of the credit card presented verbally/form and I will not dispute the payment with the credit card company; bank or financial institution in which the funds are being withdrawn as the terms indicated in this form. I understand processing of payment may take up to 72 hours after submission. I understand charges/debits will be processed using a secure processing service. Charges and Debits are NOT refundable and NOT transferable. I understand should I forfeit my appointment, service request or are more than 20 minutes late to the time and date of my reservation confirmation, I here by authorize my card on file used to secure my deposit to be charged for the remaining 70% balance and service total in full. I understand I forfeit all legal rights to dispute charges in any way or form.
